Our utmost commitment is to employ a multi-disciplinary approach in the treatment and car of our clients to effect a lasting recovery and thus restore our clients health and well-being.
Upon the completion of the initial assessment process, information gathered is used to develop a comprehensive treatment/education plan suitable to the individual client’s specific needs. This plan consists of – milestones, goals based on client’s personal life challenges and needs. The Therapy seasons are customized to the client’s needs and their unique treatment goals. This personalized attention provides a helping hand as the client walks through the path of recovery. Using the “Social Model” philosophy, we strongly encourage family involvement and participation in therapy sessions to facilitate long lasting recovery.
Case Management
Upon the initial assessment, the client is assigned to a Counselor and a Therapist who will provide individual support through the treatment cycle. The role of the Counselor and Therapist is understand the client’s treatment goals, monitor progress, coordinate client’s care and liaise with other team members to provide optimum service and ensure that clients needs are met and that treatment plans are on target. If not, a reassessment is made with the Physician’s approval to determine why not and continue address concerns and a re-plan accordingly. The case management team is ultimately charged with the management of clients care and well-being through the treatment process.
Discharge Planning
Following client’s assessment, as initial discharge plan is put in place to ensure a successful recovery goal setting. This process enables the team to work together towards the client’s recovery. The initial treatment goals set provide a bench mark with which the treatment team determines and assess progress during the course of treatment. Upon discharge, we provide a comprehensive “Alumni Support” group for the client’s ongoing needs.
